Under the theme "Plan Holistically, Then Act," the summit focused on early orthodontic intervention for children. Three distinguished guests delivered opening remarks: Professor Zhao Zhihe from West China School of Stomatology, Sichuan University; President Chen Xuefeng, President-Elect of the Private Dental Medicine Committee of the Chinese Stomatological Association; and Mr. Tian Fangjun, Chairman of Boen Dental Group. The summit featured keynote presentations from numerous experts and scholars, including Professor Zhu Min, Chief Physician at Shanghai Ninth People's Hospital, Shanghai Jiao Tong University School of Medicine; Professor Li Yu, Deputy Director of the Department of Orthodontics at West China School of Stomatology, Sichuan University; Professor Qiao Hu, Deputy Director of the Pediatric Dental & Maxillofacial Development Management Department at the Affiliated Stomatological Hospital of Xi'an Jiaotong University; Professor Liu Xinqiang; Professor Mai Zhihui, Associate Chief Physician in the Department of Orthodontics at the Third Affiliated Hospital of Sun Yat-sen University; Professor Zhao Lixing from the Department of Orthodontics at West China School of Stomatology, Sichuan University; Dr. Shen, Director of Ortho-Kids Club and renowned pediatric dental specialist from Thailand; Professor Chen Jun, Chief Medical Officer of Chongqing Weile Dental; and Dean Li Ming, Founder of Angel Dental Clinic. Over 600 attendees participated in the event.

In his opening remarks, Professor Zhao Zhihe extended a warm welcome to all guests and expressed heartfelt gratitude to the attending doctors and trainees for their trust and support. As the topic of early intervention gains increasing attention, exploration in this field continues to deepen. He expressed his hope that everyone would gain valuable knowledge and practical skills from the two-day program.

As an integral part of orthodontics, early intervention has a treatment impact that spans the entire growth and development lifecycle of children and adolescents. Professor Zhao Zhihe from West China School of Stomatology, Sichuan University, shared insights on the philosophy and practical significance of early orthodontic intervention, beginning with the three-stage application of early treatment, mouth-breathing habits, and the DOFA expert consensus on arch expansion and soft silicone aligners. Professor Zhao advised attendees to avoid overtreatment while never missing the golden window for early intervention.

— Airway Considerations in Early Orthodontic Treatment

Professor Zhu Min, Chief Physician at Shanghai Ninth People's Hospital, Shanghai Jiao Tong University School of Medicine, delivered a keynote on "Airway Considerations in Early Orthodontic Treatment." She explained that pediatric airway obstruction is a common etiology of various early malocclusions. Adenoid hypertrophy can cause nasopharyngeal airway obstruction, leading to mouth breathing, which in turn results in conditions such as narrowed dental arches and mandibular retrusion. Tonsillar hypertrophy may cause oropharyngeal airway obstruction, leading to passive mandibular protrusion and conditions such as mandibular prognathism. Airway obstruction can potentially trigger respiratory compensation, resulting in decompensation of maxillofacial development. Therefore, airway management is a critical component in preventing and intercepting early maxillofacial developmental abnormalities.

Professor Li Yu, Deputy Director of the Department of Orthodontics at West China School of Stomatology, Sichuan University, presented his unique IMSAS orthodontic strategy, analyzing clear aligner treatment approaches for Class II malocclusion through five key elements: Incisor positioning, Molar positioning, Set-up (arch arrangement), Attachment design, and Staging (tooth movement sequencing). Combined with the growth and development patterns of children and adolescents, he also analyzed the optimal timing for mandibular advancement, ensuring precise and rational implementation of treatment plans.

Professor Qiao Hu, Deputy Director of the Pediatric Dental & Maxillofacial Development Management Department at the Affiliated Stomatological Hospital of Xi'an Jiaotong University, provided a comprehensive and systematic explanation of the optimal timing and fundamental approaches for treating various malocclusions across the deciduous dentition, early mixed dentition, mid mixed dentition, late mixed dentition, and early permanent dentition stages, based on the patterns of pediatric maxillofacial growth and bone age analysis. Professor Qiao emphasized that when tooth movement is involved, clinicians must assess whether root development is complete. For jaw orthopedic procedures, it is essential to identify the peak growth period and implement appropriate treatment modalities at the right time to achieve optimal results with greater efficiency.

Professor Liu Xinqiang shared that space management is a critical component of early intervention. Through appropriate space management techniques, clinicians can guide the normal eruption of permanent dentition and eliminate factors that interfere with growth and development. Clear aligners offer better predictability and higher stability in tooth movement. When used for space management in early childhood, they leverage growth potential to precisely control eruption space, delivering high efficiency and excellent outcomes.

Using actual clinical cases, he explained the biomechanics of clear aligner orthodontics and the indications for pediatric clear aligner treatment. He placed particular emphasis on design principles and key considerations for pediatric clear aligner therapy, enabling clinicians to adopt a classification-based approach for different patient presentations.

Many complex malocclusion types in the primary dentition phase lack standardized diagnostic and treatment protocols, requiring orthodontists to conduct scientific analysis and diagnosis to achieve favorable outcomes. Conversely, inappropriate treatment can cause physical and psychological harm to patients. Therefore, orthodontic medicine must continuously advance through the integration of learning and practice, innovating scientific treatment philosophies and technologies to provide solutions for more patients.
